Overview
Potentials In Life will provide person-centered and recovery oriented psychosocial rehabilitation services to individuals with mental health problems, severe psychiatric problems, and other mental disabilities. Potentials In Life will provide Mental Health Skill Building Services in any setting in which client needs can be addressed, skills training applied, and recovery experienced.
These services include: skills training, education in accessing individual needs, and effectively utilize services and supports that are essential to meeting the needs identified in the individual service plans. Our clients will work toward their development of environmental supports necessary to sustain active community living with self-efficacy.
Eligibility
Individuals qualifying for Mental Health Skill-building Services must be age 21 and older and demonstrate a clinical necessity for the service arising from a condition due to mental, behavioral, or emotional illness that results significant functional impairments in major life activities.
The individual shall have one of the following as a primary diagnosis of:
Schizophrenia or other psychotic disorder as set out in the DSM-5
Major Depressive Disorder
Bipolar I; or Bipolar II Disorder
Any other mental health disorder that a licensed physician or licensed mental health professional has documented specific to the identified individual within the past year
The individual shall have a prior history of a psychiatric hospitalization
The individual shall have had a prescription for antipsychotic, mood stabilizing, or antidepressant medications within the 12 months prior to the assessment date.
The individual shall require individualized training in acquiring basic living skills such as symptom management; adherence to psychiatric and medication treatment plans; development and appropriate use of social skills and personal support system; personal hygiene; food preparation; or money management.
